Passport Services are available to anyone applying for a new passport, by appointment only.
MGPL does not renew passports. Renew online
Before you schedule your appointment, please note the following:
- All applicants must be present, regardless of age.
- Both parents/legal guardian(s) and children must be present for all minor applications under the age of 16.
- If only one parent/legal guardian can be present, a notarized DS-3053 form must be submitted along with a copy of the front and back of the non-applying parent/legal guardian’s ID.

Passport Requirements
Please bring the following items to your appointment. If you have any questions about what documents to bring, please call the Library at 847-929-5103 or go to the U.S. Department of State website.
- Applications may be filled out online and then printed single-sided.
- Applications filled out by hand must be filled out in black ink with no whiteout, strikethroughs, or corrections.
- Do not sign the application until asked to do so by an agent.
You must provide one of the following documents (original or certified copy)
- U.S. birth certificate that meets the following requirements:
- Issued by the city, county, or state of birth
- Lists your full name, date of birth, and place of birth
- Lists your parent(s)' full names
- Has the date filed with registrar's office (must be within one year of birth)
- Has the registrar's signature
- Has the seal or stamp from the city, county, or state which issued it
- Certificate of Naturalization
- Certificate of Citizenship
- Previous U.S. Passport
- Consular Report of Birth Abroad
You must bring a physical photo ID and a photocopy of it. If your photo ID is from a different state than the state in which you are applying, bring a second photo ID.
- Valid driver's license
- Naturalization Certificate
- U.S. passport (valid or expired)
- Government employee ID (city, county, state, or federal)
- U.S. military or military dependent ID
- Current (valid) foreign passport
You may need to show a second photo ID when presenting one of these documents:
- In-state, fully valid learner's permit with photo
- In-state, fully valid non-driver ID with photo
- Temporary driver's license with photo
- Employment Authorization Document (EAD)
Photocopies must be:
- On 8.5” x 11” standard white paper
- Single-sided
You must provide one color photo of yourself.
- Photo must be 2” x 2”, full-face, front view, with a plain white or off-white background.
- Photo must be a recent photo, taken within the past 6 months.
The Library offers Passport Photo Service for an additional $15 fee ( Cash, credit card, or check made payable to Morton Grove Public Library).
You must provide two separate payments per application.

Child Applicants (Age 15 and younger):
- Passport Book $100

You may add an expedite fee to any of these choices for an additional $60 to speed up the processing time.

Processing times begin the day your application is received at a passport agency or center, not the day you mail your application or apply for a passport at a local acceptance facility.
